Breed Survey Application
This payment includes online banking fees.
An Application for Breed Survey Form (GSDCA BS1) may be downloaded from the GSDCA website. Alternatively contact the State Breed Survey Registrar Fay Stokes on 0401 019 213. The completed form and all relevant paperwork (including fee) must be received by the registrar at least 7 days prior to the Survey date.
To apply for Breed Survey you must provide the following with your fee payment here:
- An Application for Survey Form (GSDCA BS1)
- A copy of the ANKC registration certificate (blue form) in the name of the owner. Note: animals on the limited register are not accepted for Survey.
- Copies of ‘A’ and ‘Z’ Stamp results/certificate.
- Five generation pedigree.
- For all imported animals a copy of the Title Verification Certificate as received from the GSDCA Title Verification Registrar.
- For previously presented animals that have failed survey, a copy of the GSDCA Form BS2-Notification of Failure must be supplied.

Hip and Elbow Contract
This payment is for a GSDCA Hip and Elbow Contract, including online banking fees and return a stamped addressed envelope.
Please ensure you understand all the requirements for x-raying your dog.
GSDCA HD/ED Control Scheme: Breeders and Owners Please note:
Please ensure that your veterinarian is aware of this requirement, as documented on the GSDCA HD/ED Control Scheme Application form under “Advice to Veterinarian” which reads as follows:-
“Computer Radiography Generated Images (Digital X-rays):
For computer radiography generated images (digital x-rays) to be accepted by the GSDCA Hip Dysplasia and Elbow Dysplasia Control Scheme, the images must be of a high quality and saved as DICOM Images. (i.e. .dcm extension)”

Owners of dogs submitted for HD/ED x-ray assessments (i.e. GSDCA “A” and “Z” stamp schemes) are requested not to contact the Radiologists/Readers directly regarding any queries they have about results they have received. A copy of your dog’s ANKC pedigree is required to be submitted at the time of purchasing the contract.

ZAP Application
This payment includes online banking fees.
A ZAP Application Form (GSDCA Z1) may be downloaded from the GSDCA website. Alternatively contact the Breed Affairs Officer. Contact details are on https://gsdl.info/about-us/club-contacts/
The completed form together with the relevant paperwork (and fee) must be received at least 7 days prior to the Test date.
To apply for ZAP Assessment you must provide the following with your fee payment here:
- An Application for ZAP Assessment Form (GSDCA Z1)
- A copy of the ANKC registration certificate (blue form) in the name of the owner. Note: animals on the limited register cannot be accepted for Assessment.
In this application process, the registered owner and address entered below must match that on the ANKC pedigree.
